D. Quinquevittatus

Also known as the Rio Madeira Poison Frog. The frog has black back and three narrow bluish white to greenish white stripes. The belly itself is bluish white with large black spots, while the throat and chest stripe are yellow. The arms and legs are entirely dull orange with a few small, distinct round spots widely scattered over the limbs.
